If vulvodynia is around the fourchette then it is usually due to persistent straining to achieve defecation. The lesion is perivascular nerve fiber proliferation and it responds to 1) stop straining 2) reduce pelvic blood flow - GnRH can be very helpful while the first is being achieved.

If 1) is confirmed as aetiology then avoid vestibulectomy. Refeences available.
