If you go strictly by the any BP over 140/90 prior to 20 wks gestation, we have a fair number of chronic hypertensives in our small population.

Anyone else making the diagnosis of gestational hypertension more and more? I don't know if its a name looking for a disease of if it is something that 8 years ago we simply thought it wasn't preeclampsia and it wasn't chronic hypertension so we didn't need to do anything about it, or if it is truly something to be treated aggressively.

26 YO P0000 at 36w4d, good dates. Today, had BP 145/90--reviewing prenatals, probably is a chronic hypertensive, never diagnosed, as opposed to PIH. Due to BP, I contemplated lab/BPP/bedrest/admission?/deivery, so I checked her cervix. No PIH symptoms, edema 2+, no proteinuria or excessive gain (she is 250#, give or take). She as 4/100%/bulging bag/footling. BPP 8/8, AFI 28. What's next?

A footling breech should be easy to deliver thru the C/S incision.

You really think a 26 year old is a "chronic hypertensive?"

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
