How about the cost in our time? I had a Blue Shield patient recently who wanted first trimester screening. Before I could refer her, I had to obtain prior authorization for all of the separate procedures our perinatologists needed included for first Trimester screening - genetic counseling, free beta HCG, PAPP-A, ultrasound for nuchal translucency. It took 4 days to get it authorized. There is a problem with the small window of time we are dealing with, it becomes a major roadblock for referral. If the patient comes in early enough to get screening, but by the time we get prior authorization and can schedule with the perinatologists it is past 13 wks, we've blown the opportunity for first trimester screening. The patient and her lawyers will blame us for not providing more assistance to her to get early diagnosis.

As ACOG and SMFM are promoting prenatal testing for "everyone", I was curious if anybody knows of a reference looking at the economics of this.

With the current testing available (either NT/free beta or mid-trimester serum screening) and both of their associated "false positives" (sorry to use that term). I would be curious as to the financial breakdown if we tested everybody, then acted on the positives via CVS or amnio - what would be the actual cost per aneupliody identified. Somebody must have looked at this.
