Richard,

Is the ACE Harmonic the same as the Harmonic LCS curved shears? I can't find anything on the Ethicon website about an ACE product, or is this a different company all together?

Thanks,

Glen

The trick with the ACE is to take all the tension off the vessel, whether this be the IP or the ascending uterine vessels. Hemostasis with the ACE can be affected by grasping the vessel until just before it "clicks", using the minimal energy setting for about 5 seconds (or until you see bubbling) and then removing the harmonic while still active.

Over the many years, I have used bipolar cautery, Gyrus, and harmonic scalpel.

The first two have always resulted in good hemostasis, while the harmonic scalpel caused a few anxious moments when bleeding occurred and we had to resort to bipolar to control it. I have seen this in my hands as well as others, so i am not sure it is an operator related problem (though possible)

Surveying the cyber-gyns:

Which, in your experience, is the safest instrument to take the infundibulo-pelvic ligament in laparoscopic hysterectomy?

By safest, I mean the instrument which most consistently takes this pedicle without having to go back to deal with bleeding.

In my experience it has been the seitzenger-type coagulator/cutter - Gyrus also manufactures one.

Agree? disagree?

Prefer the harmonic scalpel? Staples? Other?

Thanks in advance.
