Thank you! I have always associated functional ovarian cysts with the luteal phase vs. ovulation and was mostly puzzled by the amount of fluid. So she most likely is just an extreme case of cervical mucus production...very good!

Your patient most likely is experiencing "Mittleschmerz" or "pain in the middle of cycle." This pain can be either left or right or both. The "gush" of clear fluid is the clear, low-viscosity (about the same as egg-white) cervical mucous produced in varying degree of abundance and is seen during the day or two before and after ovulation. Neither the pain or discharge has anything to do with functional ovarian cysts.
