Your patient most likely is experiencing "Mittleschmerz" or "pain in the middle of cycle." This pain can be either left or right or both. The "gush" of clear fluid is the clear, low-viscosity (about the same as egg-white) cervical mucous produced in varying degree of abundance and is seen during the day or two before and after ovulation.
