I have a puzzle. I have a 38 year old G4P3SAB1, natural family planning patient who complains of the following for the last 2-3 years; Every other month, always on right side, at ovulation, she experiences a very sharp rectal pain ("feels like a knife is being stabbed into my rectum")that lasts about 1/2 hour! and then subsides to aching for 2-3 days. Within 6-12 hours of the pain, she has a "gush" of clear, odorless fluid from the vagina. These intermittant gushes continue for 24-72 hours, enough to require the use of 3-4 regular tampons per day. She describes it "like when my water broke in labor". Now I think she is having functional ovarian cysts, but I'm not totally conviced because of the fluid. Any suggestions? Thank you so much! The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
