'gush' of clear fluid after ovulation/ovarian cyst rupture

I have a puzzle. I have a 38 year old G4P3SAB1, natural family planning patient who complains of the following for the last 2-3 years;

Every other month, always on right side, at ovulation, she experiences a very sharp rectal pain ("feels like a knife is being stabbed into my rectum")that lasts about 1/2 hour! and then subsides to aching for 2-3 days. Within 6-12 hours of the pain, she has a "gush" of clear, odorless fluid from the vagina. These intermittant gushes continue for 24-72 hours, enough to require the use of 3-4 regular tampons per day. She describes it "like when my water broke in labor".

Now I think she is having functional ovarian cysts, but I'm not totally conviced because of the fluid. Any suggestions?
