Transverse for the skin. With regard to a preemie breech, nobody would ever fault you for doing a classical. I have always used my hand placed over the lower segment as a gauge to determine if it is "developed" enough to give me enough lateral room to make an LCT incision. If the lower segment is not "developed", it is better to chose a classical. Entrapment of the after coming head, transection of the fetal spinal cord in a preemie and "T'ing" an LCT incision are the risks of using a LCT uterine incision without adequate room. Lynn

As you know this hospital rarely does early deliveries. We had a set of 27 wk twins in labor this morning. The first was footling breech, the second transverse. We did the section and all went fairly well but we second guessed our incision choices.

What incision would you use for skin and for uterus?
