I remove obvious placenta and in particular membranes, however, I have stopped swiping around with the abdominal pack quite a while ago.

What about dilating the cervix in elective C/S? I usually pass one or two fingers and then replace the outer glove.

With regards to debris, remember these are Americans, not trained by discpiles of Joel-Cohen :-))-O

BTW, am hanging out in Pretoria until tomorrow, after snoring at the most bizarre FIGO sponsored workshop on Women's Reproductive Rights for the last two days.
