Our mentor, when we began our midwifery service back in the early 1970s (while el was still in high school), had already complete 16 years of practice in the Old Order Amish community. He always said that experience taught him things he hadn't learned in medical school, such as, that these particular grand multips weren't scary to take care of (except for a few cases; in those, he put heavy pressure on the husbands to okay tubal ligations), even though their farms were 30-35 miles from his hospital. Few hemorrhaged. Even though the Amish all live on farms and it's impossible to keep flies out, he never saw these women getting postpartum infections. This was during the same period when his hospital had to close its nursery and maternity ward more than once because of infection. It is wonderful to take care of people (like the Amish) who fully accept responsibility for their choices in maternity care. Even non- Amish in our area of Tennessee used to be that way. Dr. Williams told me about the one time he was sued. His lawyer called and said, "Doc, I've got bad news for you. It's that young fellow who was in the auto accident the other day." Dr. Williams had waited a day to observe him before operating to remove a ruptured spleen. Two days later the lawyer called again and said, "I've got good news. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
