Listeriosis

Our OB M&M had a presentation on listeriosis today. Basically it was a 25 week pregnancy, initially presented with mild symptoms consistent with UTI (and had a positive urine culture). Treated with antibiotics, rapidly progressed to severe febrile illness with IUFD. Blood cultures confirmed L. monocytogenes.

The presentation stimulated my thinking on listeriosis, and I have the following questions (I'm revising our practice handout on listeria):

1) What does "no deli meats" mean? I understand the" no processed meats, smoked fish, deli salads, soft cheese, unpasteurized dairy products, raw hotdogs, pates". What about turkey, roast beef, ham that is sliced to order?

2) In unexplained IUFD should we do blood/placental cultures to look for listeria?

3) Given the nonspecific nature of some (esp non-CNS ) listeriosis infection, what would trigger you to initiate a workup for listeriosis, and what would that workup be?
