I just did a LSTF on a 22 yo G2 P2 who came to me because her gynecologist in a nearby city would not do it. I have also done TLs on other young patients because their husband could not get a vasectomy because the urologist did not feel they were old enough. I always counsel them, as you do, regarding the risks of regret, "what if something happens to your family, your children, your marriage, etc." Ultimately, if they meet the government mandated age of 21 and are competent, I will do the procedure. In residency we used the artificial rule of 75 (age times parity had to be at least 75). It never made sense to me. If a patient is motivated, informed and competent, who am I to refuse to perform a sterilizing procedure. It comes down to autonomy and choice. Ronald E. Needless to say, this represents a small glitch, i.e. may deliver and need surgery when I'm on call. Also, it represents some division within the group, albeit very minor division which is not an issue in terms of how we get along. Any suggestions welcome. Garry

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
