Hyperpigmentation during pregnancy

I have a 27 y/o at 28 weeks who came in with a well delineated pigmented skin change on the back of both hands.

She doesn't have much else in terms of pigment changes of pregnancy - her areola are a little darker, her linea nigra is faint, she doesn't have a sacral saddle on the back of her thighs.

It looks like she was out in the sun and got a tan - but its dark and too quick to be natural.

Does anyone have any ideas?
