Was Actinomyces -- colloquialism

Gail - It's a colloquialism. What it means is that one can not win, no matter what one does. Even if you do the correct thing, someone will find fault and castigate you in some way. This is the unfortunate outcome of our wonderful legal system.

In this case, if I decide to leave the IUD in place because it does her greater good in preventing pregnancy (which is very likely to occur if IUD will be out) and she develops pelvic actinomycoses (which is extremely unlikely) it will be my clearly fault (as any attorney will be able to demonstrate to an uneducated jury (read: my peers,[ tongue in cheek ] ), and thus having done the right thing, I, nevertheless, get punished.

Hence my only way out of this predicament (and even that is fraught with failure) is to educate the patient and let her choose. It's probably the correct way out also because we should be in charge of our bodies, make our own decisions, and not have someone dictate to us what is best.
