I think you have this wrong. They were only ARM'd once they got to 3 cms and were contracting. Then if the cervix had not dilated to 4 cms after an hour syntocinon was used.

The hospital I work in sent a working party across to Dublin to see for themselves how active management worked. It lasted 2 years as the section rate was rising. (The UK national rate is 23%, ours is now 21% but it was as high as 27% when all women were actively managed. Not only that midwives felt that they were becoming obstetric nurses and so they reverted back to woman-centred care.

In Dublin at both Rotunda and NMH they have a lack of staff for the same reasons and have moved away from active management.

While I accept that active management is needed in some cases I don't think Kieran O'Driscoll's work should be taken to include ALL women. He only meant it for first labours.
