Back in the old days when I did coagulation/VTE research, we did many studies on coagulation changes in pregnancy. Fibrin degradation products were often present in normal pregnancies, and increased with preeclampsia and eclampsia. (I can remember getting frozen samples from one of the African countries that had a higher incidence of eclampsia than the US). I also spent 6 months doing these assays at King's College Hospital in London, in addition to my regular research in Boston.

However, I personally think D-dimer/fibrin degradation product is a non-specific finding, and do not order it. Nor do I order PT/PTT. By the time those tests change, the woman is so clinically ill that one can make a bedside diagnosis.

Having said that, we order, as part of our PIH profile, CBC, BUN, creatinine, uric acid, AST/ALT. If any question re proteinuria will get a 24 hour urine as well.
