It helps me know when the woman is in labor when we meet up. In our out-of-hospital situation, we have no staff. We come in when someone's in labor. If they are the same -1 station, 2 cm, firm that they were last week, I am confident that I can send them home and see them later that night if/when things pick up.

If there have been changes in her cervix, and she's been having "timeable" contractions for a few hours, she's probably in labor. It's a time management strategy for us. However, it's always an offer not a request, and it's mostly the moms that request it.

I've also seen women try to induce themselves with castor oil on an unripe cervix. If you can do a vaginal exam and tell them that it's too early and she'll only get diarrhea, they'll usually hold off and go into labor soon anyway. I'm not a big fan of castor oil induction.

In reply to: OTOH, I have had women walk around 2-3cm, 80%, -1 for weeks before labor. All the folks on the list have had the same experiences, I'm sure, so why DO you guys feel the need to check cervixes (cervices?) before labor?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
