I would simply do it, document it, bill it, and if Medicaid doesn't pay (just like they won't for cosmetic surgery), bill the patient.

You also could tell the patient it is not covered, and have them pay up front. If they still demand it and won't pay, simply tell them you choose not to do it. However, that will create ill-will and could be a slippery slope.

It may be simplest to acquiese, as these will be infrequent.

You also could come up with a "maternal anxiety" type of diagnosis, and have a preoperative second opinion. If you did that, and sent it all to Medicaid, I bet it would eventually be paid, but what a pain.
