Could call state board of health care professions and ask for guidance in delineating responsibility and soliciting its help in the situation. Also, does your instituion not have a tertiary referral institution? A call to that location with request for acceptance of patient in transfer seems useful as well. Hope you get some relief and the patient does well...also if still febrile in face of multiple gorilla antibiotic rx, consider septic pelvic thrombophlebitis.

Here in Namibia we call what the OB is doing, Desertion. Not in the face of the enemy, but it is digraceful conduct, leaving a seriously ill patient 1 day post op without arranging backup.
