We have them stay 2 days and then beyond that they only get seen if they call (ie breastfeeding issues). Otherwise it is at the 6 week postpartum issue. I'm certain there aren't many places in the states that have enough personnel to make home visits daily for 6 days after a delivery. I've pulled a bunch of review articles on GBS, but have yet to cull them to get the numbers of preventable GBS illnesses with universal GBS screening and treatment. We do test for GBS but not on everyone. If a woman comes in with any vaginal loss we swab her. If her urine had leucocytes in it we test it for GBS. We don't have a problem with GBS infection. GBS can be pos one day and neg the next so testing everyone will be futile unless you test them every day as you might always get them when they're neg. <<if they want to go home earlier then they would sign an 'Against Medical Advise" form acknowledging that we have recommended they stay and they want to go.>> And if they refuse to sign this form as our do on occasions? Our community midwives go in every day up to day 6 and then on days 8 and 10 so if any problems occur the midwife will pick it up. When your women go home what postnatal care do they receive? We also credit women with common sense as they will phone the hospital if they are worrid about anything. We don't spoon feed then or treat them like they don't know anything. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
