Cervical insufficiency vs preterm uterine contractions or both??

20 some years old G3, P1, Ab 1 (sab at 14 weeks) currently 18 weeks, twins - mild bleeding a few days ago- hospitalized - placed on antibiotics, Indocin- bed rest - Cx length 1.8 -- now at 2.45 - positive fFN. .. her fundal height is obviously > 20 weeks size - one of my partners was in favor of doing a cerclage because he believes she has cervical insufficiency - I do not favor it .. It's my understanding that she has premature contractions - my question is - would you give tocolysis e.g. Procardia now or would you go by the book and wait until she has 20 weeks ..
