All the recent recommendations are based from the one study done by MEIS and published in JEJM 1997. Basically it showed a 20-30% reduction in risk of PML in women at risk (such as your pt) when started on 17-0h Progesterone Caporate at 18 -20 weeks. It was given as an IM weekly medicaitonof 250mg. Your pt has a 30% statistical chance that she will deliver this baby prematurely. The actual cost of the drug is fairly cheap at about $10 per shot. The problem is in obtainingg the drug. I had used it before and had to lookon line to finid a distributor in New Jersey. No need to check progesterone levels before or after. My personal feeling is that she has little to loose by trying the drug and a lot to be gained in not having another preemie. I would offer it too her and explain that we just do not have enough studies yet to make it a recommendation. andrew
