Absence of fFH "predicts" the absence of labor, but it's still a crap shoot. Does it help or change your management or does it just give you a false sense of security?

Right?

THe problem is EVERYTHING is worthless in predicting preterm labor and birth. When we figure out what causes preterm labor, then we'll be able to do something about it.

Sorry, I have to jump in at this time. I teach both medical students and residents. My background is in academics and I NEVER believed in tocolytics until now. We completed our study and it will be published. We looked at symptomatic women between 24-34 wks with a +FFN and short cervix and found that if you agressively treat this group, their pregnancy is definitely prolonged compared to the group that you leave alone. However, if the cervical length is < 1.5cm, they will deliver prematurely despite tocolytics. We in Ob, have missed the boat, we have waited for digital cervical change to treat women and now we need to realize that digital exams are worthless in identifying women at risk for a preterm birth.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
