23 y/o G3, P1011 with EDC 7/5/05 (36 4/7 weeks) presents with UCs q 1-2 min. Pt. describes them as at least "an 8" on a 1-10 pain scale--she's huffing and puffing with them. She had been on Procardia 10 mg qid for PTL which was D/Cd on Thursday (36 weeks). Cx 2-3 cms/thick/Vtx -3 with no change after 4 hours. FHTs are 120-130's, reassuring. Patient and husband asking for AROM (which I refused) or IOL (which I also felt was not indicated). She refused terbutaline which the nurse had ssuggested/discussed with her prior to my arrival which I also do not feel is appropriate at 36+ weeks. U/A is normal, SG 1.1015, urine tox screen negative. She has tried resting, walking, and showering. I did both P.O. and IV hydration and even a trial of IV stadol without any change in the UC pattern. Any thoughts/comments/suggestions?? Lenora McCall, CNM

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
