I actually DID make the diagnosis prenatally - was following the patient for placenta previa when the light bulb went on and I had my Archimedes "Eureka" moment. This patient had, not placenta, but a narrow band of tissue with fetal vessels (demonstrable with doppler) covering the internal os. Her referring OB called me from the Recovery Room to thank me. (I'm usually like a broken clock - right twice a day.)

That being said - this is not an easy diagnosis to make - and the sensitivity, specificity and predictive values of the ultrasound diagnosis of vasa previa need to be worked out.
