Vulvar polyposis

40 yo P1041 s/p several years "enlarging labia", greatly increased in size last one year. Biopsy 1999 "lichen sclerosis"; 12/2004 "skin tags". Grossly gritty, polyploid-appearing enlargement of minora; polyploid growths into anus - traveling from minora across fourchette into anal opening. In our office:

1. Biopsy 3.9.05 "L vulvar bopsy-papilloma; post fourchette biopsy - lentigo, hyperkeratosis; L introitus biopsy - papilloma." 2. Pathology from a rather large vulvar excision that I helped my partner with 3.23.05(if you work with Somali women, it looked like a type III infibulation when we were done) "Vulva, labia excision - neurofibroma, patchy mild chronic inflammation; 'Rectal' polyp excision - anal mucosa with intradermal nevus, completely excised'. Note - The spindle cells in the stroma stain positively for S-100 indicating their schwannian origin."

I saw her today, and it's apparent that this polyploid process is occuring in the lines of excision - there is big, shaggy polyploid tissue in the post fourchette and by the clitoal hood on the left. She is also bleeding - she thinks from the suture in the fourchette, but we fear from the rectum (? more plyps in there.)

We are having the path re-read by dermatopathology.

Has anyone ever seen such a thing? Any ideas who would have seen it? I have pictures, but no capacity to post them. I would be glad to send them off list.
