Dear colleagues: We´ve used methotrexate as a conservative option in two cases of ectopic pregnancy, in cervical and cornual (interstitial) location. In both cases the dose was 1 mg/kg/day, and it shows as a succesfully alternative to surgical treatment, with a mild abdominal pain and minimal vaginal bleeding in the cervical. Anyway, methotrexate interferes with DNA synthesis and thus is teratogenic, so I´m not sure if an 'threshold' or 'safe' dose can be determined. Julio C Arellano, MD La Plata.
