autotransfusion in ectopic pregnancy

Dr.Capt.T.seshasai.M.D. ROLE OF AUTO TRANSUFUSION IN ECTOPIC PRGNANCY A second para, living two, regular periods with LMP on 10th march 2005 reported with pain abdomen 3 days back to surgery department. she was treated for acute abdomen with a provisional diagnosis as peptic ulcer and discharged. she had pain abdomen again and reported forlabour room at 6pm on 17th March 2005. On examination she was quite pale and tongue appears to be moist and light pink in colour. PR 82/MT. BP 70/40mm of Hg. No oedema feet. abdomen lower portion was distended and tender. PV done . Cervix rocking was tender. uterus was mobile and tender. urine was tested for urinary HCG and found positve. ABDOMENscaned free fluid was seen in pouch of douglas. uterus was normal in size. no decidual reaction was visible. Right tube was with a mass of 5cm seen. suggesting rupture ectopic pregnancy. on paracentesis the dark brown blood was aspirated of 5ml. under General anesthesia explorative laparotomy with sub umbilical mid line incision given. BLusih coloured peritonium seen. on opening we were able to collect two bags full of blood in transufusion bags, patient was on blood transfusion already started before surgery. uterus holded and a ruptured ectopic with a clot of 50grams and fresh bleeding seen 5 cm away form the fimbria. this was clamped and ligated, hemostasis secured. oppsote side tubectomy done as the patient wants to adopt Family planning permanent method. The left ovary was enlarged to the size of 5/6 cm and hemorragoc in colour. abdomen closed and the blood in the peritonium was filterd and collcet in the transfusion bags was transfused to her comfortably.
