As our former president might say, depends what "hell" is and what "handbasket" is.

Of course, the point would be, ROM at 18 weeks (implying delivery at 19 weeks) is a death sentence to the kid. So whatever you do can't be worse (for him). The chances you take are with the mother, and so you have to be VERY attentive to her and not drop any balls there. It's obviously a doctor-intensive situation, not a "here take this cerclage and call me in the morning" sort of thing.
