Whenever I admitted a patient into that residency without a vaginal Ultrasound, whether I felt it was idicated or not, my Department Head said "but, the diagnostics are not complete..."

I do one for every patient who has complaints. but but not at everry visit and not for the annual/PAP.

I have a machine so I do the level I scans. If I have a doubt or need a level 2 I send to the radiologists across town with the expensive machine :-)-O

The Namibian Medical Aids pai according to their tariff so I charge them. I don't charge patients who have Medical Aids, since there is a (more or less) guranteed payment.
