Ultrasound as part of the normal exam

I seem to remember a thought a while ago that doing a vaginal scan
during a routine pelvic exam should be commonplace.
It is not in my practice, and I don't think that many others do it
automatically for every exam.
Do any of you do them routinely for an asymptomatic person presenting,
say, for an annual examination?
If you have a reason to want an ultrasound (physical findings, history,
etc.), do you do it on the spot, or have them return, or send them
elsewhere?
If you do it on the spot, do you charge AND collect for it?
