I've done two or three of these, none of them had an associated inguinal hernia although 30% are associated with hernia. More often, I think, the mass is thought to be a hernia and it turns out to be a cyst. The ones I did were easy--skin incision over the cyst; then primarily blunt dissection to get around and posterior to the mass; identify the round ligament, if possible at both ends; and close. I think you might want to do this with a general surgeon if you think she has an associated hernia.
