GYN: ASC-US pap

44 YO P0 comes in today for opinion regarding her paps and contraception.

She has multiple sclerosis, and is relative well off/not symptomatic. She is cool on the birth control pill, but wants to be off before she has to due to the possibility of having to take MS meds which may conflict.

She had cryo years ago, unclear diagnosis (abnormal paps is all she knows). Her cervix is pinpoint and I cannot pass an endocervical brush.

11/02 Pap ASC-US, HPV High Risk +; no colpo done 2/03 Pap WNL 11/03 Pap WNL 4/04 Pap ASC-US, HPV negative

She now wants an LTL.

I also discussed Mirena, with my only concern being pain of insertion (and difficulty, too) due to prior cryo in a nulliparous patient.

How would you handle her pap and contraceptive needs?
