dypareunia

28 yo G1P1 who have a fairly traumatic delivery, presented to me about a year ago for severe posterior introital dyspareunia. There was scar tissue which I removed, and at the same time tried to open the introitus, but no luck. Patient still is complaining of the pain, and when she uses either an over the counter 'numbing' medicine, or Lidocaine jelly, she will rub the area raw.

Any suggestions? I was things something like a mediolateral relaxing incision, as her perineal body is about 1 cm in diameter. She has tried dilators. Apparently her husband is very well endowed, but refuses debulking surgery.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
