The attitude that an IUD should be dealt with is a good example of the bad paternalistic attitudes that were so rightly criticised in the 70's an 80's by women's rights groups. It is absolutely forbidden in my department to do anything with an IUFD until all the details have been discussed with the patient and in particular emphasising the need to think about the possibility that one needs to have time to come to terms with the loss of the baby. I usually encourage them to go home for a week and return to discuss it further, and I also tell them that I do not want to do anything based on medical need for 6 weeks. Any other way of handling this is unfair and fails the patient - we should be the dispassionate professional giving the best advice and to fail to tell them about the grieving process is inadequate. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
