Has she ever been pregnant or lactated premenopausally? There are anecdotal reports of grandmas nursing their grandchildren in third-world societies when mom dies in childbirth. I would imagine she would be able to lactate after giving birth to twins, though i can't imagine anyone having the energy to do so after giving birth to twins at age 54, presumably by c-sec. Yikes! I'm 5 years younger and just the thought of it makes me tired.

Find her a good lactation consultant.
