Excluding a urinary fistula (very unlikely unless you performed also a vaginal hysterectomy) and taken for granted that the patient really has post-operative genuine stress incontinence (and not a form of detrusor instability), I simply believe that the operation has failed. It happens. I would wait 3 to 6 months and than I would perform a second TVT if stress incontinence persist. I recommend urodynamics before reoperation.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
