Pelvicol Dermal Implant During Pelvic Reconstruction

Was wondering if:

1) Anyone is using the Pelvicol porcine collagen matrix for their A/P repairs, etc. and if so, what has been your impression? Any erosions, problems or complications? Any other collagen type matrix you prefer and why?

2) How are you billing this? Are you using a 22 modifier for increased complexity of the procedure and if so, are you getting paid by Medicare/others?

I have been using this specific porcine dermal implant for anterior/posterior colporrhaphy with directed defect repair in my patients, primarily those with poor tissue/collagen and have been impressed with the postop results, no significant problems to date, but a limited series, to be sure. As A/P repair is essentially a hernia repair, use of a graft/mesh (akin to our general surgery brethren) seems to me both appropriate and a likely evolution of reconstructive surgery for pelvic hernias in the next few years if results continue to be encouraging. A literature search on dermal implants in this regard yields little results and I would be interested in the experience of the many respected members of this forum in this technique.
