Have performed about 800 LSH's, and post hysterectomy bleeding encountered maybe 2%. Most patients unhappy with unpredictable spotting or bleeding and thus trachelectomy was the Rx of choice by most pts. Despite specific discussions and instructions (written and verbal) to pathology for multiple sections to find residual endometrium, none was ever found in these trachelectomy patients.All LSH pts. neeed to be forwarned pre-operatively about this possibility. A word of caution!!! When cauterizing the endocervical canal after LSH, be certain the cautery is restricted to the upper residual canal itself. Passing the cautery forceps through the exocervix may result in significant thermal injury to the vagina. JGB
