The only time that you can justify billing both the TV and TA is when both are needed to complete the exam, e.g. huge uterus that is well above the pelvic brim. However, if you can't get a good enough view with the TA and then do the TV or vice versa, billing for both is fraudulent. I can't remember the last time that I needed info from both to be able to complete the study. I'm almost amazed that your payers would reimburse for such an approach. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
