She came in today and stated that she has been cramping since the placement of the IUS. I'll take that for symptoms.

Simply removing the device will clear the actinomyces. The choice is either take the device out and make alternative contraceptive arrangements, or leave it in and treat with antibiotics. If she is or becomes symptomatic it is better to remove it, but there is no need to waste all that penicillin on a 6 month course. Steve
