Mirena expulsion

I have had some dificulty with pulling them out when I take out the inserter. If one only pulled it down a little so that the bottom end was in the canal and not out so it could be seen,, I wonder?????

A little off the original subject, but last week I placed a Mirena in a nulliparous woman. Things went well until last night when I had to remove it in the ER because she was expelling it, accompanied by a lot of pain. Have never seen this occur and am inclined to blame it on nulliparity. Reviewed old obgyn messages on the subject and didn't see any similar problem.
