The strip is designed so that the edges are unidirectionally once you pull it through its very hard to pull out again, basically you can tighten the sling but its difficult to losenen it. Joe take out an anatomy text and have a look there really isn't anything there to be worried about. Once the needle enters just lateral to the pubic ramus its always pointing and traveling in a medial direction away from the vessels and nerves of the obturator foreamen, the idea is to meet the needle tip with the index finger of the vaginal hand.
