Since I left the US Army Medical Corps (more like "waltzed out" of Ft. Sill, OK almost 30 years ago), I wouldn't have been able to articulate as well as what you've just done in response to Leonora's issue raised about supervision. The military is, or was, a unique place to practice OB-GYN, and as in other spheres, there are both good and bad features to this setting. Gerald P. It is a serious situation when a O3 doctor (Captain in the Army or Air Force, Lieutenant in the Navy for you civilians out there) has to command and evaluate an O5 or O6 CNM that may also be the Department Head, or even facility commander, in medical matters only but in no other sphere of operation. There is simply not an analogous civilian situation that I can think of. I can attest from personal experience that a CNM with a Silver Chicken may not be even remotely interested what a lowly O3 MD has to say about his or her medical practice, and has a variety of ways of making that lack of interest clear. This creates the potential for one of those classic "responsibility without authority" situations to play out. I have always enjoyed working with CNMs, but pretending that a rank differential in a military setting is inconsequential is disingenuous to say the least. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
