This doesn't answer the original question about what defines the start of labor, but I recently had a similar case as Betsy's. A primip had been progressively dilating with no labor. She and her husband were very adament about no intervention (ironically, he finished law school this May). Finally at 40.5 weeks she was 6-7 cm/0 station. The difference here was that she was +GBS. After much lengthy (and heavily documented) discussion, they agreed to go to the hospital for antibiotic prophylaxis and amniotomy, since they were against pitocin. I broke her water 4 hrs after the antibiotics were started. She had about 10 min of painful contractions, pushed another 10 min and had her baby.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
