BRAIN FREEZE! "Now that delivery is complete, CYTOTEC would be a likely
choice" - as you asked. Rupture is no longer a concern (at least a priori
[since she delivered by c/s we know the cause of persistent pp hemorrhage
is not occult rupture]). tj

Let us pray Luis isn't listening! Have You any data on "Causality" with
thrombocytopenia and bronchospasm? Tho bronchospasm in asthmatics was
significant with PGF analogues, they are extremely rare with PGE's. I'd be
more worried about the hematologic effect of postpartum hemorrhahge than
that of cytotec. Now that delivery is complete, hemabate would be a likely
choice for ecbolic Rx when pit alone is insufficient. tj

You would have to be careful with asthmatics and preeclamptics because
cytotec can cause bronchospasm and thrombocytopenia.

Does anyone have any experience with Cytotec and postpartum hemorrhage
in women that have just undergone a c-section. I know that Cytotec is
contraindicated in women with a previous c-section for labor induction.
I have used cytotec rectally for women with vaginal deliveries that have
postpartum hemorrhage.
The reason this came up is we have a postpartum c/s that is diabetic,
asthmatic, preeclamptic with a postpartum hemorrhage. Methergine is
out. Hemabate is out. Pitocin was running. Pt is now several hours
out from surgery. Any other suggestion?
Thanks,
Glen
