Yes, this happens with my new residents about once a year and I now include a mention of this in the tutorial on caesarean sections; how to recognise and avoid it. I've also seen the posterior lip of the cervix turn up and be mistaken for lower edge of uterine wound! Gerald P. Rodríguez wrote:

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
