Another trick I've done is to reach SUPERIORLY and pull the youngster's feet first. Deliver him/her breech through the uterine incision and GENTLY pull the head up out of the pelvis. I think this avoids the trauma of grunging a hand down under the vertex and trying to dislodge it.

P.S. Of course, when you document you delivered a vertex by breech extraction via C/S, you get some double takes. LOL
