Majority of fetuses with NT of 4.5 mm ( about 80% in one series) will be normal. Remember to scan for other anomalies at 20 wks and also obtain fetal echo as well. Formal genetic counseling by someone familiar with the import of increased nuchals is highly recommended. also - if these are monochorionic twins, the possibility of a twin - twin transfusion syndrome should be kept in mind here. good luck.
