How does one diagnose insulin resistance? Unless you are doing a insulin glucose clamp study, you are only guessing at resistance. You can measure hyperinsulinemia, but many insulin resistant patient have normal insulin levels. Go back and read the original papers. The findings suggest if the androgens are elevated and cycles irregular, metformin works well. I now utilize metformin in almost all WHO typeII patients as first line therapy and only add in clomiphene or letrozole after a minimum of three months of therapy including a low glycemic diet.
